1. How big is the pond? (gallons?width?length?depth?)

2. How many fish are in the pond? (what size?)
4. Describe the waterchanges or top-ups interval and amount(if any)?
5. How does the water look to you? (clear?green?tea colored?)
6. What's the temperature of the pond?(
please use a thermometer)
7. What type of filter do you have? (biofalls?skimmer?canister?etc.)

8.What size pump(s) are in the pond? (flow?gph?)
9. Has the pump been off for any reason?
10. Did you get new fish or plants?
11. Were those fish or plants kept in isolation prior to introduction?
12. If so, what treatments were done in isolation? (chemicals?products?)
13. What fish in the collection are affected? (old or new fish?)
14. Have you tested your water quality? (pH, hardness and nitrate at least)
15. What signs, marks, or symptoms do the fish show? (hiding?sores?blood streaks?)
